From time to time photos of athletes competing at OFSAA Championships may be published on Championship websites, the OFSAA Bulletin, newspapers and/or other OFSAA publications.

Please make your student athletes and their parents/guardians aware that when participating in an OFSAA Championship there is potential for an athlete’s picture to be taken and posted on championship websites, the OFSAA Bulletin, newspapers and/or other OFSAA publications used strictly for OFSAA purposes.

If there are athletes on your school team who do not wish to be photographed please fill out the information below.

OFSAA will do their best to prevent your athlete from being photographed however does not assume responsibility if the situation occurs.

I hereby certify that all athletes on _________________ team competing at the OFSAA _____________________ Championship are aware and authorize use of their photographs for the Ontario Federation of School Athletic Associations with the exception of those listed above, and its contracted vendors (photo/ video/ media publications) in all forms of media and in all manner, including electronic media and/or composite representations, for advertising, trade, or any lawful purposes and waive any right to inspect or approve the finished product, including written copy that may be created in connection therewith.
