Checklist for Coaches! Entry forms can be completed on your internet browser; however, you will not be able to save the form as a file to email, unless you have Adobe Writer. To complete forms do the following: Type the information in the appropriate sections. Print out the completed typed form. Acquire all appropriate signatures. Fax the completed form with appropriate signatures, to the attention of Jim Barbeau at (613) 967-1963 IMPORTANT: If you download a form and the page is blank and/or generates errors, you probably need to upgrade your version of Adobe Acrobat® reader.
